Return and Refund Policy

Thank you for choosing PAR 5 Physical Therapy.

If, for any reason, You are not completely satisfied with your physical therapy, we invite you to review our policy on refunds and returns.

The following terms are applicable for the physical therapy treatment sessions that You purchased with us.

The words of which the initial letter is capitalized have meanings defined under the following conditions. The following definitions shall have the same meaning regardless of whether they appear in singular or in plural.

For the purposes of this Return and Refund Policy:

* Company (referred to as either "the Company", "We", "Us" or "Our" in this Agreement) refers to PAR 5 Physical Therapy, LLC, 300 Main St Ste 21 #662 Madison, NJ 07940.

* Service refers to Physical Therapy Sessions and Treatment Packages.

* Website refers to PAR 5 Physical Therapy, accessible from <https://www.par5pt.com/>

* You means the individual accessing or using the Service, or the company, or other legal entity on behalf of which such individual is accessing or using the Service, as applicable.

Your Refund Rights

You are entitled to a refund of your Service within 3 sessions without giving any reason for doing so. The 3-Session Guarantee is a satisfaction guarantee and not a progress guarantee. Medical progress and change in your medical status (pain or dysfunction) are not guaranteed. If the level of service and attention to care is not to your satisfaction, you may request a refund under the conditions set forth below. The deadline for canceling a Service is 30 days from the date on which You purchased the Services or on which a third party you have appointed, who is not the carrier uses the Services delivered.

In order to exercise Your right of cancellation, You must inform Us of your decision by means of a clear statement. You can inform us of your decision by:

We will reimburse You no later than 14 days from the day on which We receive the notification. We will use the same means of payment as You used for the purchase of the Services, and You will not incur any fees for such reimbursement.

Conditions for Refunds

In order for the Services to be eligible for a return, please make sure that:

* The Services were purchased in the last 30 days.

* You have followed the treatment plan and home exercise program recommendations set forth by your physical therapist in good faith.

* You have attended your sessions within the frequency as directed by your physical therapist in good faith.

* You have notified your physical therapist of any complications in your treatment at each session.

* You have notified your physical therapist of any medical complications or past medical history that may affect your progress.

We reserve the right to refuse refunding of any services that do not meet the above refund conditions at our sole discretion.

Only regular-priced Services may be refunded. Unfortunately, Services on sale cannot be refunded. This exclusion may not apply to You if it is not permitted by applicable law.

Gifts

If the Services were marked as a gift when purchased and then given directly to you, You'll receive a gift credit for the value of your Services.
